Where to find the Education Reimbursement Form?
1) From the AKN, log into My Allina

Once logged in, look to the right of the page and look for the heading called "Popular Topics"
3) Click the topic "Forms"
4) Look for the form called "Tuition/Education Reimbursement"
5) This will open in a new window, print this form and fill out. Once class is paid for
send form along with proof of payment to HR. Reimbursement will be made through paycheck. Also note, that proof
of coarse completion is required or funds will be deducted from your payroll.
CERTIFICATION BONUS
Mercy
and Unity MNA contract employees--remember that Mercy and Unity pay a $400 bonus once a year (in October)
for each certification held, up to two separate certifications. So, if you already have a certification (CCRN,
CMC, CSC, CCNS, SANE-A, etc.) consider getting a second certification for another $400 bonus! Take the course now, sit
for the test this summer, receive $400 in October!
Non-Mercy/Unity MNA contract employees--check with your HR department
and/or your unit manager to determine if your facility offers a certification bonus.
